The likelihood of success for the client dramatically increases as changing former habits and creating new habits also creates possibilities and is a process done as a team.Coaching is for those who are emotionally, and psychologically healthy and who want to create positive and lasting changes in their body composition, attitude about food, and life performance and move forward with the action to create improvements. Coaching is guidance but not advice, therapy or counseling. By entering this relationship, the Coach and Client acknowledge that the client wants to make significant progress and changes in his/her life at this time. Because progress and change happen at different rates that are unique for the individual, the client and coach both commit to staying in regular contact with each other to navigate the road to the client’s goal together. Ultimately, the client understands that the progress that is made is dependent on his/her actions and attitudes only, as the coach has no ability to “change” anyone else, as her role is guide the client through the change processes.Client Initials *I understand the Coach’s CommitmentWhat Your Coaching Program Includes: *12 weeks of online lessons, workshops and optional workouts delivered daily to the clients mailbox and is accessible via mobile or desktop devicesIndividual Coaching Sessions (3 Hours per 3 months) (or 1 hour for the Limited Version) ideally via Skype or Zoom to discuss planning, goal-setting and strategic and personalized planning (can be divided into 30 min blocks to increase coaching frequency). The Coach and Client commit to starting on time, and if the client is more than 10 mins late for a coaching session, the Coach will assume the session is cancelled and the client will forfeit the session. (24 hour notice required for appointment changes)Any time email and Facebook Group supportPrivate Feedback and interaction through the online coaching program and email between the coach and clientClient Initials *I have read and understand the coaching program componentsConfidentiality: Since the coaching relationship is built on trust, the Coach and Client agree to keep all conversations and information shared with one another private and confidential. No personal ideas, information or thoughts expressed will be shared with anyone else except with the permission of the coach and or client or both. *Client InitialsCoaching Fees and Guarantee: By opting into the payment plan, you understand and agree to being billed every 30 days for the duration of your program. *Client InitialsI agree to pay the amount as indicated on my initial enrollment form. Payments will be charged to whichever form of payment was used for initial enrollment payment unless an alternate payment is provided from client to coach. Client will receive a full refund providing the following stipulations are met: *Client completes 80% of the lessons, habits, workshops and feedback requests from the online program;Client attends all 3 private coaching hours (or 1 hour if on Limited Program) during each 12 weeks session.Client understands that the online program will not be paused or restarted. The Coach only expects the clients best efforts to participate, not perfection.
